On every visit, for every shift, our CCAs are required to complete a Daily Field report which focuses on observations, measurements, and a check list of activities that require monitoring according to our team of geriatricians and gerontologists and our Plan of Assistance and Care. These detailed reports and their summaries are available on a real time basis on our clients’ Family Portal. In addition, our CCAs may take photos of our clients, solely to make them available on this private, secure, HIPPA compliant family portal. This enables our clients’ family members to have remote access as well as a photographic history of special events and moments, activities and other occasions of note.
The Compass Care Family Bridge also contains a private library which houses, in one simple, secure and private location: all service contracts; complete Client Care Team listing with detailed biographical and contact information; past and current service schedules; medication management schedules; as well as clinic treatment and care information regarding that specific client’s conditions. By melding state-of-the-art technologies with advanced, evidence based care interventions, measurements and protocols, we enable our clients’ families to you to remotely share in and monitor their loved ones’ activities and progress.
